This NP will be required to have and maintain a valid driver's license. This NP is expected to make home visits and prioritize conducting tele-video visits with Veterans during the Pandemic. The HBPC NP acts as the provider for patients admitted to the HBPC Program. He/she is responsible for interviewing, evaluating and assessing, diagnosing and treating acute care patients in their home. NURSE PRACTITIONER (NP) for HOME BASED TRANSITIONAL CARE - Work Schedule:

This position can be located at any of the following locations:
Bellevue CBOC, Clermont CBOC, or Florence CBOC. This HBTC NP is accountable to the Home Base Primary Care Nurse Manager. The HBTC NP works in collaboration with the Home Based Primary Care Medical Director, who is responsible for the overall medical care delivered by the HBTC team. This NP will be driving a government vehicle in performance of HBTC duties. This NP will be required to have and maintain a valid driver's license. This NP is expected to make home visits. This NP acts as the provider for patients admitted to the Home Based Primary Care Program. The experienced NP is responsible for interviewing, evaluating, assessing, diagnosing and treating frail, chronically ill, or disabled Veterans in their home. The provider leads the HBTC interdisciplinary team providing care to patients, ensuring the complex medical, psychosocial, rehabilitative, behavioral or palliative care needs are met; ensuring workload of the team is equitable and manageable. Other duties and responsibilities include but are not limited to the following:
assumes primary medical responsibility of assigned patients; conducts a comprehensive assessment of patient's medical, physical and biopsychosocial status obtaining a relevant health and medical history; performs in-home physical examinations based on age and history; conducts preventative screening procedures based on age and history; updates evaluation of health status to determine need for further health care; formulates appropriate medical diagnoses based on data obtained and defining the medical management; Orders appropriate diagnostic procedures and laboratory tests; identifies and prescribes appropriate pharmacological and non pharmacological agents; identifies the needs of the individual and family based on the evaluation of collected data; determines the need for, and facilitating admission to, the VA medical facility and other transitions of care; consults with appropriate collaborating health care professionals; determines the effectiveness of the plan of care through documentation of client care outcomes continually reassessing and modifying the plan of care as necessary to achieve medical and health goals; collaborates with community partners via phone or in-person to facilitate safe and timely transition into home; utilizes telehealth technology in providing care to patients. The provider will coordinate patient care via telehealth technologies throughout the continuum of care. Both of the NP positions listed above are skilled health care providers utilizing critical thinking and judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacological and non-pharmacological treatments in the direct management of acute and chronic illness and disease. The provider's practice promotes wellness and prevents illness and injury. This NP utilizes preventive clinical guidelines, appropriate standards of care, and collaborates with the Home Based Primary Care Medical Director in providing quality care to patients presenting with acute and chronic conditions. Nurse Practitioner (NP) for a Patient Aligned Care Team (PACT) - Work Schedule:

This position will be stationed at the Florence KY CBOC however travel between main campus and/or CBOCs will be required on occasion. This NP is assigned to a PACT where the patient is the center of a care team that includes family members, care givers, and healthcare professionals - a primary care provider, a nurse care manager, a clinical associate, and an administrative clerk. This NP is accountable for the management and evaluation of a panel of patients under his/her care. This NP provides comprehensive preventative and wellness care, as well as the spectrum of acute, episodic, chronic, and end-of-life care to Veterans assigned to his/her PACT. Basic Requirements:
United States Citizenship. Education - Possess a master's degree from a program approved by the appropriate State-accrediting agency and accredited by of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). Licensure - Current, full, active, and unrestricted license, or otherwise recognized, as an Adult Nurse Practitioner in any State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Qualifications:
At least 1 year of Nurse Practitioner experience is preferred. At least 2 years of Nurse Practitioner experience in the following areas is preferred:
treatment of congestive heart failure, chronic obstructive pulmonary disease, etc. At least 2 years of Nurse Practitioner experience working in an inpatient setting, acute care setting or home care setting as Provider is preferred. Experience as a Provider directing patient care in the home setting is preferred National Certification related to area of assignment Active DEA License and State controlled substance registration Current BLS and ACLS certification Demonstrated leadership, self-direction and rational problem-solving skills Ability to work effectively with clinical medical equipment Documented evidence of excellent communication skills verbally and in writing Basic computer skills Excellent interpersonal and organizational skills Works well with interdisciplinary team Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree. Reference:

Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. Regarding this specific RN position the population of male and female veterans served ranges from adult, age 18, to the elderly adult, age 65 and over. The VA Medical center is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continues walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. BLS/ACLS REQUIREMENTS UPON SELECTION:
Per VHA Directive 1177, current certification in BLS issued from the American Heart Association or Military Training Network is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high-risk areas. If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification (issued from the American Heart Association or Military Training Network) prior to being boarded for an appointment with the VA. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
